Yes, one of our qualified providers who has extensive experience with seniors will become your primary doctor. In addition to your primary care physician, you will be assigned an Interdisciplinary Team of health care specialists, including nurses, physical therapists, social workers, registered dietitians, and more.

With our PACE program, you are in good hands, as we contract with Mount Sinai Medical Center physicians and other highly rated community physicians right in your neighborhood. Our contracted hospital is Mount Sinai Medical Center.

Mount Sinai Eldercare will provide transportation to all of your appointments and to the center. This transportation is provided at no cost to our participants.

Most of our participants enjoy Mount Sinai Eldercare at no cost.

There are no co-pays, deductibles, or hidden fees of any kind, even prescription medications from the Mount Sinai Eldercare physician are covered. Mount Sinai Eldercare participants receive all of their health care services, including primary care and specialty physician services (except for emergency services), from Mount Sinai Eldercare or a provider authorized by the program. Participants may be fully and personally liable for the cost of unauthorized or out-of-PACE program agreement.

Services provided at the center, including Day Health Care, are based on the needs of each participant and will be included in your care plan.
Yes, Mount Sinai Eldercare will ensure you have access to food and meal programs if you require assistance.
Mount Sinai Eldercare provides dental, hearing, and vision services right at the Mount Sinai Eldercare Center. Each participant will see these specialists shortly after enrollment and at least annually thereafter. If medically necessary, Mount Sinai Eldercare will provide hearing aids, glasses, and dentures.

Mount Sinai Eldercare provides in-home services, so there is no need to keep your current services. Your individualized care plan will include the home care services you need.

As soon as you are enrolled in Mount Sinai Eldercare, our Interdisciplinary Team will assess your needs and begin services right away.

Our enrollment process takes approximately 6-8 weeks. Our enrollment specialists will guide you every step of the way.

Once you enroll in Mount Sinai Eldercare, we become your insurance provider. Our all-inclusive coverage provides the medical and social services you need, and our plan has a dedicated team of health care professionals working with you to promote health and wellness and support your desire to remain at home. Please note that Mount Sinai Eldercare participants must agree to receive all of their health care from Mount Sinai Eldercare or one if its network providers (except for emergency care). In addition, Mount Sinai Eldercare participants may be fully and personally liable for the cost of unauthorized or out-of- Eldercare program agreement services.
Mount Sinai Eldercare is a voluntary program, and participants can disenroll at any time.

No, the goal of our program is to help seniors live safely at home.

While encouraged, it is not a requirement to attend the Mount Sinai Eldercare Day Health Center from week-to-week. Attendance is based on the participant’s needs. However, regularly scheduled doctor appointments and other visits with your Interdisciplinary Team occur at the Mount Sinai Eldercare Center.
