Eldercare Program Overview

Welcome to Mount Sinai Eldercare.

Mount Sinai Eldercare is a federal- and state-sponsored Program of All-Inclusive Care for the Elderly (PACE). We offer comprehensive medical, social, and recreational services to adults over the age of 55 who are living with a chronic illness or disability and need coordinated medical care to continue living as independently as possible at home or in the community.

PACE is truly all-inclusive care. The PACE program provides seniors with primary medical care, specialty doctors, medical imaging, restorative therapies, rehabilitation, nutrition, social work, home care, prescription drugs, medical equipment and supplies, mental health services, transportation, and much more. Mount Sinai Eldercare’s services are offered in our PACE center, at home, or in the community.

Our new center is conveniently located at 6050 West 20th Avenue in Hialeah. It was designed to offer you (or a loved one) all the services you need in one convenient location, from medical care, therapy (physical, occupational, and speech) services, dietary assistance, and social services. Meet new friends, participate in stimulating activities, and enjoy delicious and healthy meals.

The Mount Sinai Eldercare Center is open Monday through Friday, 8 a.m. to 5 p.m. We are pleased to be able to offer our community access to a new facility and expert care team. We welcome the opportunity to provide tours for those who are interested in learning more about our comprehensive program.
