At Mount Sinai Medical Center’s Braman Comprehensive Cancer Center, we provide specialized care for patients with several forms of blood cancers, including chronic leukemias, indolent and aggressive lymphomas, and multiple myeloma. Blood cancers, also called hematologic malignancies, impact the blood, bone marrow, and lymphatic system. While some forms progress slowly and require careful monitoring, others are highly aggressive and require immediate therapy. Your health and peace of mind are our top priorities. Chronic Hematologic Cancers

Chronic hematologic cancers develop gradually and often require ongoing monitoring and individualized care. These conditions include chronic leukemias and indolent (or slow-growing) non-Hodgkin lymphomas (NHL), which may progress slowly but still require expert management to optimize long-term outcomes and minimize complications.

Unlike many acute cancers, some chronic leukemias and indolent NHLs are generally considered long-term, manageable diseases rather than a condition that can be permanently cured with standard treatments. Current therapies are highly effective at controlling the disease and managing the symptoms rather than achieving a cure as many patients can live full, active lives for many years without requiring immediate therapy or intervention.

At the Braman Comprehensive Cancer Center, our care combines advanced diagnostics, targeted therapies, and access to innovative clinical trials help patients achieve durable remissions, effectively manage their symptoms, and maintain the highest possible quality of life.

Chronic Lymphocytic Leukemia (CLL)

Chronic lymphocytic leukemia (CLL) is the most common type of adult leukemia in the United States, with around 21,000 new cases diagnosed annually. Classified as both a leukemia and a form of non-Hodgkin’s Lymphoma (NHL), CLL is a malignancy of B-cell lymphocytes, a type of white blood cell. The disease is characterized by a progressive accumulation of mature lymphocytes—monoclonal or genetically identical white blood cells that build up in the blood, bone marrow, and/or lymph nodes. Over time, this accumulation crowds out healthy blood cells and weakens the immune system.

The initial clinical presentation of CLL is highly heterogeneous. While many cases of CLL progress slowly, some patients may present with immediate complications that require urgent intervention such as severe anemia (low hemoglobin), thrombocytopenia (low platelets), and immunosuppression.

Treatment Approach

Our strategy for treating CLL focuses on precision and maintaining patients’ well-being:

  • Immunotherapies help the body’s immune system recognize and fight cancer more effectively.
  • Active surveillance allows some patients to enjoy treatment-free time while maintaining a good quality of life.

Hairy Cell Leukemia (HCL) and HCL Variant

Hairy cell leukemia (HCL) is a rare, slow-growing chronic leukemia affecting B lymphocytes. HCL and HCL variant (HCL-V) are distinct, rare, and chronic splenic B-cell lymphomas/leukemias that partially overlap in how they appear and are diagnosed but differ in genetic basis, prognosis, and management. More than 95% of the patients with HCL have the activating BRAF mutation, V600E. Detection of this mutation is useful in confirming the diagnosis of HCL and determining treatment options.

With hairy cell leukemia, abnormal cells accumulate in the blood, spleen, and bone marrow, which can interfere with the production of normal blood cells and lead to a weakened immune system. Symptoms may include fatigue, shortness of breath, swollen lymph nodes, and frequent infections.

For patients with HCL or HCL-V, our team develops individualized treatment plans that can include chemotherapy or the use of targeted therapies to control the disease while minimizing side effects. These therapies have made it possible for many patients to live nearly normal life spans.

Many important questions still remain unanswered in hairy cell leukemia. More research is needed to elucidate outcomes in specific disease subtypes and complications of the diseases and their therapies. At Mount Sinai, we are part of an international HCL Registry, a critically important tool that enables researchers to study hairy cell leukemia in ways not previously possible given the rarity of the disease. Through the Registry, we cooperate and collaborate with patients, doctors, and institutions around the world to collect HCL patient data. The Registry helps researchers identify new trends in patient outcomes, discover previously unknown complications of the disease, and design clinical trials for new therapies until a cure is discovered.

Indolent and Aggressive Lymphomas

Lymphoma is a type of cancer that affects our lymphatic system, a vital part of the body’s immune system that helps the body fight infections. It develops when lymphocytes grow and multiply abnormally, and it can involve the lymph nodes, blood, bone marrow, spleen, and virtually any organ or tissue throughout the body, such as in the gastrointestinal system, central nervous system, bones, and skin.

Lymphomas are broadly classified based on how they behave and how quickly they grow. Indolent lymphomas are slow-growing and often progress gradually. Many patients with these lymphomas do not require immediate treatment after diagnosis and can be safely monitored until therapy becomes necessary. In contrast, aggressive lymphomas grow more rapidly and typically require prompt evaluation and treatment. Although they progress quickly, most of these lymphomas are highly responsive to therapy, and timely treatment can lead to durable remissions and, in many cases, a cure.

At the Braman Comprehensive Cancer Center, we specialize in the diagnosis and management of both indolent and aggressive lymphomas. Our multidisciplinary team including hematologists, pathologists, radiologists, radiation oncologists, nurses, and supportive care specialists work collaboratively to ensure that every patient receives an accurate diagnosis and develop individualized treatment plans tailored to each patient’s disease and goals. This collaborative approach allows us to deliver timely, evidence-based care while supporting patients throughout every stage of their treatment journey.

Through our clinical research program, eligible patients have access to innovative clinical trials for both newly diagnosed and relapsed or refractory lymphoma. These studies provide opportunities to receive promising new treatments that may not yet be widely available while contributing to advancing the future of lymphoma care.

Myeloma

Multiple myeloma is a chronic blood cancer that affects plasma cells, a type of white blood cell responsible for producing antibodies that help fight infections. In this disease, abnormal plasma cells accumulate in the bone marrow and produce large amounts of dysfunctional antibodies, known as monoclonal proteins (M proteins) or paraproteins. These abnormal proteins can potentially damage bones and organs such as the kidneys, and lead to anemia, fatigue, frequent infections, elevated calcium levels, kidney dysfunction, and other complications.

Personalized care for patients with multiple myeloma focuses on delivering targeted therapies to effectively control the disease while minimizing side effects. Our hematology team recognizes that multiple myeloma, like other chronic conditions, requires a long-term, comprehensive approach to care. We are committed to ensuring that every patient receives the right treatment at the right time while providing ongoing monitoring for potential complications, including infections and secondary cancers. Through continuous, coordinated care, we strive to improve outcomes and help our patients maintain the highest possible quality of life.

Clinical Trials

At the Braman Comprehensive Cancer Center, patients with blood cancers (lymphoma, CLL, hairy cell leukemia) have access to observational and interventional clinical trials that help advance the field forward, including access to novel targeted therapies. These studies help advance the field, offering patient early access to therapies that may not yet be commercially available. We guide patients through every step of the process, from enrollment and informed decision-making to ongoing monitoring throughout the study. In addition to advancing medical research, participation in a clinical trial empowers patients to take an active role in their care and treatment decisions.

Our clinical research program includes studies across multiple phases, from early-phase trials that evaluate the safety and effectiveness of new therapies to Phase 2 and Phase 3 studies that compare promising treatments with current standards of care. All studies are conducted under rigorous scientific and ethical oversight to prioritize patient safety while helping advance the development of future treatments.
