Genitourinary Reconstructive Surgery (GURS) Fellowship

Position Summary

The Mount Sinai Medical Center GURS Fellowship is a 1-year, high-volume, advanced surgical training program in male genitourinary reconstruction. The fellow will train directly with Dr. Billy CordonDr. Michaela Sljivich, and Dr. Scott Polackwich, gaining comprehensive expertise in urethral reconstruction, prosthetics, trauma reconstruction, genital surgery, men’s health, robotic reconstruction, and upper tract reconstructive surgery. The fellow will function with autonomy expected of a junior attending, with graded independence in the OR, and clinic. Additionally the fellow will be expected to complete research projects during their time here.

Core Clinical Responsibilities

Reconstructive Urethral Surgery

  • Anterior and posterior urethroplasty
  • Radiation-induced strictures
  • Anastomotic, augmented, and staged repairs
  • Buccal mucosal graft harvests
  • Reconstruction after trauma, pelvic fracture, or iatrogenic injury
  • Rectourethral fistula repairs
  • Gracilis Flap Harvest

Male Incontinence & Prosthetic Surgery

  • Artificial urinary sphincter (AUS)
  • Male urethral sling surgery
  • Inflatable penile prosthesis (IPP) placement, revision, and salvage
  • Management of prosthetic complications
  • Complese revision prosthetic cases

Genital & Penile Reconstruction

  • Peyronie’s disease surgeries (plication, grafting, IPP with corrective procedures)
  • Penile trauma repair
  • Buried penis repair (simple and complex)
  • Scrotal and perineal reconstruction
  • Split-thickness grafting

Trauma & Emergency

  • Urologic trauma evaluation and management
  • Penetrating trauma to urethra, penis, scrotum, and bladder
  • Blunt trauma to GU organs
  • Posterior urethral distraction defects
  • Testicular rupture, torsion, and high-acuity genital injuries
  • Priapism Management

Robotic & Upper Tract Reconstruction

Robotic Reconstructive Surgery

  • Robotic ureteral reconstruction (simple & complex)
  • Robot-assisted ureteral reimplantation
  • Robotic pyeloplasty (primary and redo)
  • Robotic ureterolysis for retroperitoneal fibrosis
  • Robotic ureteral stricture repairs with buccal graft
  • Robotic ureteral reconstruction after iatrogenic or radiation injury

Upper Tract Reconstruction

  • Ureteral stricture evaluation and management
  • Endoscopic vs. reconstructive decision-making
  • Complex Ureteral Reconstruction Robotic vs Open

Men’s Health & Andrology

  • Erectile dysfunction management
  • Hypogonadism and testosterone therapy
  • Aesthetic and functional genital procedures

Operative Expectations

  • Fellow will focus on reconstructive operative procedures.
  • Fellow will operate 3-4 days per week.
  • Will need to meet GURS requirements and submit case logs at 6 months and 12 months.
  • Fellow will function as a junior faculty

Outpatient Responsibilities

  • Fellow will spend 1 to 2 days a week in clinic
  • Will work up reconstructive patients from preop to postop and will learn to follow reconstructive patients longitudinally.
  • Fellow will become proficient in various outpatient proceudres including: RUG, VCUGs, UDS, Penile dopplers, Xiaflex injections, TRUS, dilations

Research & Academic Duties

  • The fellow will have dedicated research time ½ day per week (Tues/Wed afternoons)
  • Completion of at least one manuscript suitable for publication
  • Presentation of work at AUA, GURS, SMSNA, or other national meetings
  • Participation in outcomes research and clinical databases
  • Teaching residents and medical students

Call & Coverage

  • Will enter the faculty call pool and take primary faculty call.
  • If available will cover trauma/reconstructive call.

Teaching Responsibilities

  • Hands-on teaching in OR, clinic, and simulation labs
  • Resident and medical student instruction in reconstructive techniques

Professional Development

The fellow will graduate with mastery in:
  • Complex urethral reconstruction
  • High-level prosthetic and incontinence surgery
  • Robotic and open upper tract reconstruction
Will have honed skills in : Speaking, presenting research projects, and writing manuscripts

Qualifications

  • Completion of an ACGME-accredited urology residency
  • Eligibility for unrestricted Florida licensure
  • Demonstrated interest in reconstructive urology and robotics
  • Professionalism, maturity, and strong surgical skill
